It has no correlation with the effort that's put into the stroke. He's not the only one that does this, but it certainly seems to be the most blatant example of this. For all the criticism we might direct towards Sharapova and Azarenka, the one thing I'll say about both of those is that they are scrupulously consistent in that regard. There are big names I won't mention who aren't in my opinion, but there it is. Some say it's part of the game, but I for one can't stand it..
