I wonder if the guy has shoulder issues...I know I do. I can hit every shot properly but serving hurts like crazy.

I don't know what to say. That might work on the Futures, even Challenger, circuit, but ATP? Even a top 50-100 would hammer those. Imagine what Rafa would do.

The Chang one against Lendl was pretty memorable. Chang did just about everything he did to unsettle Lendl in that match - moonballing, standing right up to the box on receiving serve... anything to unsettle Ivan, and it worked.
I've seen McDonald play. He has a regular serve. He must have been suffering from a shoulder injury that day.
I do think throwing in an underarm serve once in a while could work on the WTA.
